What might strike a reader as a serious omission or noticeable incompleteness might readily be explained by the editor with the statement that the material in question would be found in another volume. Many topics in ophthalmology might with appearance of propriety be indexed under several different letters of the alphabet. One writer would place them under one word, another under a totally different heading.
